This policy or notice is provided by Velatura Aesthetics, PLLC, doing business as Velatura Medical Aesthetics & Wellness (“Velatura,” “we,” “us,” or “our”).
Velatura is a self-pay practice and does not bill health insurance or send electronic claims to health plans. Your medical records are confidential under Florida law (section 456.057, Florida Statutes). This notice explains how we protect them and what you can ask of us.
Your rights
When it comes to your medical records, you can:
- Get a copy of your records. You, or your legal representative, may ask for copies of your medical records at any time. We will provide them within the time Florida law requires. A reasonable copying charge may apply, but we will not withhold your records because of an unpaid balance for services. We will verify a representative’s authority before releasing records.
- Ask us to fix a mistake. If you believe something in your record is wrong or incomplete, tell us and we will review it and respond.
- Raise a concern. If you believe your privacy was not protected, contact us using the details below. You may also file a complaint with the Florida Department of Health. We will never retaliate against you for raising a concern.
Marketing and selling information
We will not use your medical records to market goods or services to you, and we will not sell your health information, without your specific written permission. You may revoke any written permission at any time, except where we have already acted on it.
How we use your information
We use your health information to treat you and coordinate your care, to process self-pay transactions, collect amounts owed, and issue receipts, and to run the practice — scheduling, quality, training, and required record-keeping.
We do not share your medical records with anyone else without your written permission, except where Florida law requires or allows it — for example, a valid subpoena, a Department of Health inspection, care coordination with another treating provider, or required reporting such as suspected abuse.
Our responsibilities
We keep your records confidential as Florida law requires, and we train our team to do the same. If a breach ever compromises your personal information, we will notify you as required by law.
